For all sports we require online registration through our partner company, FinalForms.
EVERYONE PLAYING AN ELPS SPORT MUST REGISTER USING ONLINE REGISTRATION
In order for your student to participate and be included on a coaches roster, registration must be completed and approved. FinalForms is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FinalForms, the system keeps track of your information in your FinalForms account so you enter your information only once for multiple uses, multiple family members and multiple programs.

Before You Register
Review the Athletic Handbook
It will be helpful to have the following information handy to allow for accurate completion of your online registration:
- An electronic copy of most recent physical
- Doctor information
- Health Insurance information
- Emergency contact names and phone numbers
- If your student requires medication - a completed Authorization for the administration of Medicine form
PHYSICAL EXAMS: Student Athletes must have a current (within the past 13 months) Health Assessment form (physical) on file in the registration software to be able to participate in practices and or games. If the student’s physical expires during the season, they will not be able to play until a new physical exam is received. We DO NOT accept "bridge letters" since we give the athlete 13 months from the date of the last physical. During registration you will be required to upload the physical to complete registration.
